Pegasus Application/Update Form

Please complete this application form if you have a disability or impairment and you would like to register for the Pegasus scheme to make it easier for you to contact us in future. 

You can also complete the form if you are already registered for Pegasus and want to tell us that your details have changed.

You can, if you wish, give the details of one or two people you can trust, who we can contact if we need to in an emergency. Please think about who you would like to nominate and check with them if they are willing to be your nominated trusted person for the Pegasus scheme. 

The information you give on this form will be used to update the Pegasus database. We will share your information with the other emergency services (Nottinghamshire Fire and Rescue and East Midlands Ambulance Service) if you say that you allow us to. We will not share your information with anyone outside these emergency services without your permission.
